After dealing with Mas90 tech support (Sage), I discovered a handy little "cache" checkbox under ODBC settings for SOTAMAS90. Dirty Read and Burst Mode are also selected.
Before discovering the above settings, support asked me to Reinitialize the Crystal temp tables for Sales Order with the following steps:

Library Master

Repair
-
Reinitialize Data Files
-
Choose module and company code
-
S/O Crystal - yes
Crw - yes
We also tried a new form code. This is the trick. If a new form code works, then Mas90 is fine and there is either something wrong with the previous form or it's code.
